How fees work
Orthodontics is a longer course of care than a cleaning, so here is how paying works, in the order most people ask.
Insurance we work with
Many dental plans include orthodontic benefits, and what they pay varies by plan, by age and by whether the plan covers adults at all. We file claims for you with plans that cover orthodontics. We are not enrolled with Medicaid or CHIP.
PPO plans with orthodontic benefits: we file claims
DHMO plans: ask us before you book

If you pay yourself
You get a written estimate before treatment and choose how to pay. Financing is offered by third-party lenders. Approval and terms are set by the lender.
Pay as we agree in writing

Receipts for reimbursement
After each payment we give you an itemized statement. If your plan reimburses part of orthodontic care, you can send it in; the plan decides what it pays.
Itemized, for every payment
Procedure codes on request
An estimate in advance
You can ask for a written estimate before any treatment. If you're paying without insurance, we give you a written estimate in advance, when you schedule or whenever you ask. It lists the records visit, the treatment we expect and its cost.

What is and is not included
Take it home before you decide
Checking your benefits
Before your first visit we contact your plan and tell you what it says about orthodontic benefits for you. We can't see everything a plan will decide later, so we tell you which parts are theirs to confirm.

What the first visit includes
A talk about what you want, an exam by Dr. Jafari and the records she needs, then her plan for you and a written estimate. Fill in your health history on our private patient portal, not by email or in the contact form.
